Getting Started
Before scheduling an initial appointment, I offer a brief phone consultation. This gives us an opportunity to discuss what brings you to therapy, address any initial questions you may have, and determine whether it makes sense to schedule a first meeting.
Your First Appointment
Our first meeting is an opportunity for us to begin getting to know one another, giving us an opportunity to consider whether working together feels like a good fit. We will talk about what brings you to therapy, what you hope to work on, and any relevant aspects of your history and current life circumstances.
Forms
Prior to your first appointment, please complete this packet of forms, which can be signed electronically and emailed. Alternatively, you may download and print them in advance of your first appointment.
Rates/Insurance
Please contact Dr. Ben-Hanania to discuss rates for services. Please note that I am an “out-of-network” provider and do not participate on insurance panels. I will provide invoices and receipts that can be submitted for out of network reimbursement if offered by your insurance plan. This out of network reimbursement guide may be helpful in navigating this process.
Cancellation Policy
Your appointment time is reserved specifically for you. If you need to cancel or reschedule, I ask that you provide at least 24 hours' notice. Appointments cancelled with less than the required notice, as well as missed appointments, are generally charged the full session fee.
Confidentiality
Confidentiality is an essential part of psychotherapy. What you share in therapy is treated as private and confidential, and information about your treatment will not be disclosed without your written authorization except in circumstances where disclosure is permitted or required by law.
There are certain legal and ethical exceptions to confidentiality, which we will review as part of the informed consent process before beginning treatment. I encourage you to ask questions at any point if you would like to better understand how your privacy is protected.
